A newly released report from the city's Department of Buildings shows a litany of elevator problems at the Manhattan high-rise were an ad executive was crushed.
According to the DOB, 285 Madison Avenue received 11 violations against the 13 elevators in the building in the aftermath of the deadly accident.
Over the past 10 years the building was hit with 56 violations.
The elevator that malfunctioned last month did not have a "certificate of compliance."
Other elevators had more serious issues.
Marketing executive Suzanne Hart, 44, was crushed when an elevator suddenly shot up as she was getting on back on December 14.

Investigate the DOB, have the feds run it. how many more people have to die?
